Benefits Add Dependents-Beneficiaries Screen

Use the Add Dependents/Beneficiaries screen to set up or modify the dependents and beneficiaries associated with your benefit plans, such as medical and life insurance.

General

Field Description
First Name

Enter the first name of the dependent/beneficiary.

Middle Name

Enter the person's middle name.

Last Name

Enter the person's last name.

SSN/Government ID

Enter the person's Social Security number or government ID number.

Relationship

Use this drop-down list to select the relationship of the person to you (Husband, Wife, Son, Daughter, and so on).

Other Relationship

This field is enabled and required when you select Other from the Relationship drop-down list. Use it to enter a relationship different from any of the choices in the Relationship drop-down list.

Gender

Select the gender of the person from the drop-down list.

Type

Select the appropriate description for this person:Dependent Only, Beneficiary Only, or Dependents/Beneficiaries.

Status

Select the Student or Handicapped check box, or both, if applicable.

Date of Birth/Agreement

Use the calendar lookup to select the date of birth (or the date of a legal agreement/document, such as an adoption agreement) of the dependent/beneficiary.

Address

Field Description
Same Address as Employee

Select this check box when the dependent/beneficiary's mailing address is the same as yours. The address fields will be disabled.

If you need to update your address, go to Self-Service  > Employee Information  > Address/Phone.

Street 1

Enter up to a 30-character, alphanumeric street address, line 1, for the dependent/beneficiary.

Street 2

Enter up to a 30-character, alphanumeric street address, line 2, for the dependent/beneficiary.

Street 3

Enter up to a 30-character, alphanumeric street address, line 3, for the dependent/beneficiary.

City

Enter up to 25 alphanumeric characters for the dependent/beneficiary's city.

State/Province

Use the drop-down list to select the state/province for the address. The valid values for states/provinces are set up in Costpoint for the selected country.

Postal Code

Enter up to a 10-character, alphanumeric postal code for the dependent/beneficiary.

Postal Code Formatting

A display shows how the postal code should be entered, if you are required to use a specific format. Format rules are set up on the Self Service Configuration screen.

Country

Use the drop-down list to select the country. The valid values are the countries set up in Costpoint. If you make a different selection in the Country field, the State/Province drop-down box will repopulate with valid values based on the country selected.

Telephone

Field Description
Home

Enter up to 25 alphanumeric characters for the dependent/beneficiary's home telephone.

Office

Enter up to 25 alphanumeric characters for the dependent/beneficiary's business telephone.

Cancel

Select Cancel to return to the Dependents/Beneficiaries screen without saving any changes.

Reset

Select Reset to undo any changes that you have made.

Save

Select Save to save changes and return to the Dependents/Beneficiaries screen, which will display the updated record.
